Please feel free to ask any of the staff if you would like to have a meeting with the pharmacist to talk about your medicines.

The meeting will take place in private in our consultation room and all aspects of your medicines will be discussed. You will find out exactly what your medicines are for, why you need to take them and if you are taking them correctly.

You can mention any problems that you are having with your medicines this could include side effects, difficulty swallowing, forgetting to take medicines or problems ordering your medication. The pharmacist will help you to sort out these problems at the review meeting.

After the meeting, you will receive an Action Plan form filled in by the pharmacist, to summarize any important points that have been discussed.
